LEGENDS YOUTH BASKETBALL ASSOCIATION Waiver, Release of Liability, and Acknowledgment of Risk Program: Fleming Island Legends I, the undersigned parent or legal guardian, voluntarily register my child to participate in Fleming Island Legends offered by Legends Youth Basketball Association ("Legends YBA"). In consideration of my child's participation, I agree as follows: 1. Assumption of Risk. I understand that basketball and related activities involve inherent risks, including but not limited to contact with other players, falls, sprains, fractures, and other injuries. I knowingly and freely assume all such risks, both known and unknown. 2. Release and Waiver of Liability. On behalf of myself, my child, and our heirs and assigns, I release and hold harmless Legends YBA, its directors, officers, coaches, volunteers, sponsors, and facility hosts from any and all claims, demands, or causes of action arising from injury, illness, death, or property damage connected with participation in Fleming Island Legends, except to the extent caused by gross negligence or willful misconduct where such limitation is not permitted by law. 3. Medical Authorization. I certify that my child is physically able to participate. I authorize Legends YBA staff or volunteers, in an emergency when I cannot be reached promptly, to obtain medical treatment for my child at my expense. 4. Code of Conduct. I agree that my child will follow coach and staff instructions and treat teammates, opponents, officials, and families with respect. Legends YBA may remove a participant whose behavior is unsafe or disruptive, without refund when permitted by program policy. 5. Photo and Media Release. I grant Legends YBA permission to use photographs or video that include my child in legitimate organizational communications (website, social media, newsletters, and promotional materials). Legends YBA will not sell images to third parties. Contact info@legendsyba.com to opt out in writing before the program begins. 6. Electronic Signature. I agree that typing my name or signing electronically carries the same legal effect as a handwritten signature. I have read this waiver, understand its terms, and sign voluntarily.
